Aşınmış dişlerde farklı bir protetik yaklaşım: olgu sunumu

Abstract


Excessive tooth wear occurs with dental hard tissue destruction due to pathological reasons. The etiological factors may be mechanical, chemical and psychological, or their combinations. Dental attrision creates serious problems with masticatory and speech functions, and esthetics of the patients. Excessive occlusal wear can also cause loss of vertical dimension. This serious loss of tooth structure, requires a comprehensive restorative treatment.

45 years old female patient was admitted to our clinic with complaints of loss of aesthetics and masticatory function. First of all, the root canal treatment of the upper front teeth -all of which is vital- was performed. Then the crown lengths were extended by prefabricated posts and composite cores. The patient's occlusal vertical dimension increased by restorations of temporary acrylic crowns. The patient has been made to use these acrylic temporary crowns for 3 months to get used to the new vertical dimension. At the end of this period, the patient's upper and lower teeth were restored with metal-ceramic crowns. An occlusal splint was made for protective purposes.

 

ÖZET

Aşırı diş aşınması, dişin sert dokusunun patolojik nedenlerle yıkımı ile oluşur. Etyolojisi  mekanik, kimyasal ve psikolojik etkenler veya bunların kombinasyonları olabilir. Diş aşınmaları, hastaların çiğneme ve konuşma fonksiyonlarında ve görünümlerinde ciddi problemler doğurur. Aşırı oklüzal aşınma dikey boyut kaybına da neden olabilir. Bu ciddi diş yapısı kayıpları, kapsamlı restoratif tedavi gerektirir.

45 yaşında kadın hasta, estetik ve fonksiyon kaybı şikayeti ile kliniğimize başvurdu. Hepsi vital olan, üst ön  dişlerin önce kanal tedavileri yapıldı. Ardından prefabrik post ve  kompozit korlarla kron boyları uzatıldı. Hastaya oklüzal dikey boyutunu yükselten geçici akrilik kron restorasyonları uygulandı. Yeni dikey boyuta alışabilmesi için akrilik geçici kronlar hastaya 3 ay kullandırıldı. Bu sürenin sonunda hastanın alt ve üst dişleri metal destekli seramik kronlarla restore edildi. Koruyucu amaçlı oklüzal splint yapıldı.
